Key Facts About Red Blood Cells (RBCs)

Production rate: About two million new RBCs are made every second to replace those lost.

Lifespan: RBCs live for about 120 days. Old cells are removed by the spleen and liver, and new ones are made in the bone marrow.

Shape: RBCs have a dimpled, disc-like shape that helps them carry oxygen and slip through even the tiniest blood vessels (Fig. 1.2).

Colour: Their red Colour comes from haemoglobin, an iron-rich protein that binds oxygen—blood is bright red when oxygenated and darker when oxygen-poor.

No nucleus: Mature RBCs lack a nucleus and organelles, allowing more space for haemoglobin, but this also means they cannot repair themselves and must be replaced.

Blood typing: Proteins (antigens) on the RBC surface define your blood group (A, B, AB, O), which is essential for safe transfusions.

White Blood Cells (WBCs)

White blood cells (WBCs), also known as leucocytes, are the body’s soldiers, form­ing the front line of your immune defence against harmful microorganisms and foreign substances such as toxins or allergens. Produced in the bone marrow and lymph nodes, they detect, attack, and destroy infections throughout the body. There are several different types of white blood cells, and each has a specific job in pro­tecting you (Table 1.1).

Platelets

Platelets, or thrombocytes, are small, irregular cell fragments that make up less than 1% of blood volume. They originate from large bone marrow cells called megakaryocytes and lack a nucleus, which is why they are not considered full cells (Fig. 1.3). Despite their small size (about 2–3 μm in diameter), platelets play a vital role in blood clotting (haemostasis) and wound healing. They circu­late in the bloodstream for approximately 7–10 days before being removed by the spleen.

Platelets also play a vital role in inflammation and wound healing by releasing proteins and signalling molecules that promote tissue repair. A normal platelet count ranges from 150 to 450 × 109/L (billion per litre of blood). Platelet counts can also vary naturally during normal life stages, such as pregnancy, where a mild reduc­tion—known as gestational thrombocytopenia (seen in about 10–15% of pregnan­cies)—is considered a normal finding.

However, when platelet counts fall outside the normal range (typically 150,000–400,000/μL), they can indicate the following:

If moderately low (50,000–99,000/μL), you may bruise easily or bleed for longer than usual. This can be caused by infections, autoimmune diseases, bone marrow problems, or certain medications.

If severely low (less than 50,000/μL), there is a greater risk of spontaneous or internal bleeding, which may require medical attention.

If too high (greater than 450,000/μL), you may be at risk of forming abnormal blood clots. This can occur due to inflammation, infection, or bone marrow disorders.

If you allow fresh blood to clot and then remove the clot, the remaining clear, yel­lowish fluid is called serum—it is essentially plasma without clotting factors and is commonly used in many blood tests (Table 1.2).

Plasma or Serum: Which One Is Used in the Blood Tests?

The choice between plasma and serum depends on the type of test and the compo­nents being measured.

Plasma or Serum: Which One Is Used in the Blood Tests?

The choice between plasma and serum depends on the type of test and the compo­nents being measured.

Blood Serum

 

Plasma: Preferred for tests that need clotting factors or the full range of blood components, such as coagulation studies, electrolyte analysis, and some hor­mone assays. Plasma is obtained by centrifuging blood that has been treated with an anticoagulant, which prevents it from clotting.

Serum: Used when clotting factors are not needed or might interfere with other tests, such as in antibody tests, many hormone tests, and biochemical assays. Serum is obtained after blood has clotted and been centrifuged.

Novel Uses and Opportunities for Serum and Plasma

Serum and plasma are increasingly being used in new and innovative ways in mod­ern healthcare. Liquid biopsies—using blood or other body fluids—now allow doc­tors to detect and monitor diseases like cancer by identifying circulating tumour DNA (ctDNA) and other biomarkers without invasive procedures. These biofluids are also becoming important in personalised medicine, where their molecular information helps tailor treatments to an individual’s unique biology. As research advances, serum and plasma continue to play a growing role in diagnostics and disease understanding, improving patient care and medical insight (see also Chap. 10).

Helpful Tips: Serum Versus Plasma

Serum is simple to collect and widely used for many diagnostic tests.

Best for: routine chemistry and serology.

Limitation: lacks clotting factors, so it cannot be used for coagulation studies, and clotting/haemolysis may affect results.

 

Plasma contains clotting factors and offers a more complete picture of blood components.

Best for: tests that assess your body’s clotting ability, as well as advanced laboratory studies that analyse a wide range of blood proteins and other molecules.

Limitation: requires anticoagulants and careful handling, making it more complex and costly.

Blood Groups

When someone says their blood type is A, B, AB, or O, they are referring to specific antigens found on the surface of their red blood cells. These antigens determine a person’s blood group and are crucial because the immune system can react to unfa­miliar antigens, making accurate blood type matching essential for safe transfu­sions. Each person inherits a unique combination of these antigens from their parents. Although more than 30 different blood group systems have been identified, the most well-known is the ABO system, which includes four main blood types: A, B, AB, and O.

In addition to the ABO system, the Rh factor (positive or negative) further clas­sifies blood types. These systems are especially important for safe blood transfu­sions, organ donation, and pregnancy care, for example, to prevent haemolytic disease of the newborn

The ABO Blood Group System

The ABO system is the most well-known and widely used system for classifying blood. It is based on the presence or absence of two antigens (molecules which can trigger an immune response) on the surface of red blood cells: A and B. There are four main blood types in the ABO system:

Type A: Has A antigens on red blood cells and makes antibodies against B.

Type B: Has B antigens on red blood cells and produces antibodies against A.

Type AB: Has both A and B antigens and does not produce anti-A or anti-B anti­bodies. It is known as the universal recipient (for red blood cells).

Type O: Has no A or B antigens but makes antibodies against both. It is known as the universal donor (for red blood cells), since there are no A or B antigens to trigger a reaction.

The Rh Factor (Rhesus Factor)

In addition to the ABO classification, red blood cells may or may not carry another antigen called the Rh factor (specifically the D antigen). This creates another layer of classification:

Rh-positive (+): You have the Rh antigen on your red blood cells.

Rh-negative (−): You do not have the Rh antigen on your red blood cells.

So, combining ABO and Rh systems gives eight possible blood types: A+, A, B, B, AB, AB, O+, and O. For example, someone with type A− blood has A antigens on their red cells but lacks the Rh factor.
